Eden Brown synergy are currently recruiting for Staffordshire County Council for the position of Senior Social Worker – Discharge to Assess (D2A) Team.
* Location: Staffordshire (Cannock and Stafford)
* Hours: 37.5 hours per week
* Band: 6
* Start Time: 08:00 – 17:00 (with 1‑hour lunch)
* Contract Length: 4 months – may extend if required
* Line Manager: Vanessa Cotton – Operational Service Delivery Lead, Discharge to Assess West
Responsibilities
• Facilitate timely discharges and prevent delayed transfers of care for all Staffordshire patients placed in a Discharge to Assess bed (D2A) in private care homes as commissioned by MPFT in Cannock and Stafford. • Work closely with other Multi‑Disciplinary team members (Social Workers, Social Care Assessors, Nurses, Therapists, MPFT Home First Team) to forge partnership working relationships across various hospital and community sites. • Promote timely, safe discharges from hospital and prevent delayed transfers of care, and support individuals in the community to prevent readmission, assess need for ongoing care, and complete timely reviews, supporting individuals remaining at home whilst promoting ongoing independence. • Conduct Statutory Social Care Assessments, Mental Capacity Assessments, Best Interest Meetings, and attend Continuing Healthcare Decision Support Tool Meetings.
Qualifications
• Experienced Level 2 Band 6 Social Worker. • Working knowledge of the Care Act (2014), Mental Capacity Act (2015), and experience conducting s.42 enquiries. • Team player with excellent communication and IT skills, capable of working autonomously and under pressure.
