Job overview


· The post will be based in Quarella Road with strong links to Local Authority, the inpatient mental health wards at Angelton Clinic, our GP partners and third sector organisations.

· The post holder will hold a first level qualification in mental health nursing and have excellent clinical knowledge and experience of mental health and the Mental Health Measure 2010. Excellent communication, listening and organisational skills are essential along with MDT working, in a challenging and changeable environment.

· To deliver evidence based holistic professional mental health care to older people within the context of an integrated multi-disciplinary and multi-agency community mental health team.

· The post holder is identified as a skilled and creative practitioner with knowledge of working with and supporting people living within their own communities and will be an integral member of the Community Mental Health Team.

· The post holder will work collaboratively with the wider multi-disciplinary teams to meet the needs of people within a community setting.

This post is fixed term/secondment for 6 months to cover maternity leave. If you are interested in applying for it as a secondment, please obtain permission from your current line manager prior to applying.

· To hold a clinical caseload providing comprehensive assessment, care and treatment to clients referred to the Community Mental Health Team in accordance with Part 2 of the Mental Health (Wales) Measure 2010.

· Acting within their professional boundaries, the post holder will provide support and intervention through holistic person-centred assessment, clinical consideration, diagnosis, treatment and evaluation of care within a multi-agency team approach. The post holder will demonstrate safe, clinical decision-making for older people within their homes.

· Foster robust, open and positive working relationships with all partners in particular Local Authority and other mental health services and the 3rd sector, ensuring that all partners are treated with dignity and respect.

· Proactively contribute to the continuing improvement and development of community mental health services, within an environment that welcomes new ideas and listens to everyone

· The post holder will frequently work as a Lone Worker.

Welsh Skills Desirable: This post is advertised as Welsh Desirable. This doesn't mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing candidates. This isn't 'fluency', just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health.
