Service Support Worker - Assisted Discharge
Location: Winchester Hospital - occasional cover to support Basingstoke and North Hampshire Hospital
Hours: 21 per week. 2 week rota, 10am - 6pm and 12pm - 8pm
Contract type: Fixed Term Contract Until 31st March 2026 with possibility of an extension.
Salary: £22,932 (£13,759 for 21-hour week) per annum, pro rata
Driving Requirement: Full UK Manual Driving Licence Required (held for a minimum of 2 years), to drive British Red Cross vehicles
What you’ll do
* Providing practical and emotional support (telephone calls, welfare checks, shopping etc) and guidance to service users
* Providing reactive, focused, and flexible support to adults following a hospital stay to enable early discharge
* Responding to referrals from Clinicians and Community Health Professionals
* Assess service users\' needs, complete and follow a support plan for the individual
* Supporting service users in the weeks after hospital discharge or to prevent readmission
* Taking someone to a hospital appointment or assisting with prescriptions and shopping
Requirements
* Have a full UK manual driving licence
* Right to Work in the UK; sponsorship is not available for this role
* Good knowledge of NHS and Social Care services
* Ability to improve service quality for the benefit of users
* Professionalism with diplomatic and confidential handling of queries
* Flexibility in working hours
There is no personal care or administration of medication for this role.
