Generic Occupational Therapy & Physiotherapy Support Worker
Band 4 – 2 Posts (37 hours/week, Permanent)
This is an exciting opportunity to join the Discharge to Assess (D2A) Team in Moray as a Generic Band 4 Occupational Therapy & Physiotherapy Support Worker. The D2A team is therapy-led, operational 7 days a week since 2021 and works alongside social care colleagues in START (Short Term Assessment & Reablement Team) and a range of other services. Together, we support early discharge from hospital, enabling patient assessment in their own homes with a rehabilitation focus. The goal is to maximise safety and independence in daily living activities through focused, goal‑oriented assessments that promote self‑management and overall wellbeing.
This role is part of a wider expansion supported by Scottish Government funding to enhance Moray’s delivery of the NHS Grampian Home First and Unscheduled Care Improvement programmes.
Key Responsibilities
* Promote the team to community groups
* Educate patient groups
* Provide treatment and adjust programmes within scope of practice
* Collate evaluation data for the team
* Hold a full, current driving licence and have access to a vehicle insured for business use
