Job overview

An exciting opportunity has arisen to join the Addictions Team on a 6 month secondment!

This role will support the clinical nurse specialists in the alcohol team and the advanced nurse practitioners in the drug team to deliver personalized care the the patients on the caseload of the team.

The successful applicant will be a co-facilitator for the SMART recovery group and offer an in-patient fibroscan service for appropriate patients. They will maintain a caseload and work closely with wider members of the addiction team and the treating tobacco dependency team.

This secondment opportunity is for 30 hours per week, with a 8-4 working pattern over 4 days. It would be suitable as a developmental opportunity for applicants currently in a Band 3 patient-facing role.

Main duties of the job

Advisors provide flexible, personalised support to inpatients to help them to reduce harm from problematic drug and alcohol use during their admission. The postholder will have excellent communication skills and be able to quickly build trusting and collaborative relationships with patients. The post holder will deliver specialist motivational and behavioural support, develop personalised care plans in partnership with patients and facilitate access to appropriate pharmacotherapy and psychological support within the wider Addictions team. This will include support at the weekly behaviour change support group in the hospital – SMART Recovery.

This role will contribute to tackling health inequalities and focuses on engaging with people whilst admitted as an inpatient. This includes offering in-patient fibroscans for patients with multiple health disadvantages who find it hard to attend out- patient appointments. The postholder will provide onward referral to community services to enable patients to receive wrap around support on discharge.

The postholder will support the development and day to day running of the service to ensure that patients receive safe, evidence-based care. They will be responsible for acting as an alcohol and drug champion, promoting the harm reduction message through their day-to-day work as a member of the multidisciplinary health care team.

Care/Service Provision
• To manage own caseload and liaise with team members in relation to ongoing reviews of treatment plans made by the specialist team
• To provide a reactive fibroscan service to patient with health inequalities who find it hard to attend OPA
• Review inpatients who are need ongoing support following an assessment by the drug or alcohol specialist nurses. Offering supportive and motivating conversations and treatment.
• Provide specialist behavioural and motivational support as part of personalised care plans.
• Refer patients to community services to continue with relapse prevention support attempt.
• Develop personalised support care plans including recommending treatments based on assessed needs.
• Provide general health, lifestyle advice and support to people using the service.
• Participate in educating inpatients about the benefits of making changes to their drug and alcohol use whilst in hospital and upon discharge.
• Use careflow to record interventions and manage tasks, keeping accurate records to enable good data collection. Always maintain up-to-date and comprehensive records.
• Ensure that the relevant equipment is provided and maintained on an agreed schedule. Report any failings of equipment or other environmental factors to the clinical lead.
• Respond effectively to referrals within stated timescales.
• Be aware of how to report incidents, concerns or complaints and escalate any concerns or complexities to the clinical lead.

Monitoring and Administrative Tasks
• To contribute to the monitoring and evaluation of the service, including the completion of the minimum data set requirement,
• Maintain accurate patient records in line with Trust guidelines.
• Ensure administrative duties are carried out efficiently and effectively to meet the needs of the service.
• To be involved in the monitoring and evaluation of patient/client feedback, offering recommendations as part of the team to improve services.

Person specification

Qualifications and Training

Essential criteria

1. One of the following: • Level 3 RQF, SCQF, City and Guilds 3, NVQ 3 or BTEC National Diploma in Health and Social Care • Or equivalent experience (minimum of 12 months)
2. GCSE grade A- C (grade 4 and above) or equivalent in English
3. GCSE grade A- C (grade 4 and above) or equivalent in Maths
4. Qualified and experience in using FibroScan

Desirable criteria

5. Qualification in motivational interviewing/ supporting behavioural change

Work Experience

Essential criteria

6. At least 12 months experience of working in a health or social care setting
7. Recent experience of working with patients in an NHS, acute, community or relevant setting within the last 2 years
8. Experience of communicating health information and advice using a person-centred approach.
9. Ability to work independently and as part of a team.
10. Experience of providing healthcare, healthcare advice, health promotion advice interventions on a one-to-one basis or as part of a group setting
11. Experience of working directly with patient with drug and alcohol dependency

Knowledge/Skills/Abilities

Essential criteria

12. A good level of understanding of evidence-based practice in relation to problematic drug and alcohol use, the health and wider consequences of drug or alcohol and benefits to modifying behaviours
13. An understanding of human behaviour in relation to making behaviour changes and decisions
14. The ability to build and maintain effective working relationships with all professionals within the multidisciplinary team
15. Ability to act, where appropriate, as the patient advocate to ensure that dependency treatment needs are met during an inpatient stay and on discharge to the community
16. Evidence of excellent listening and communication skills (both written and verbal)
17. The ability to quickly build rapport and sustainable therapeutic relationships with patients with varying health needs and from a variety of socio-economic backgrounds
18. Good organisational skills and the ability to prioritise workload without direct supervision
19. Accurate record keeping
20. Good standard of computer skills

The successful applicant(s) will normally commence at the minimum of the scale unless they have previous NHS service at the same band. Progression through the scale is by annual increments.
