Remote Practice Nurse
Days: 3 to 4 days PW
Hours: 8am - 5pm
SystmOne - Must have own laptop with S1 access or able to collect from practice.
Main duties: providing telephone and video consultations to patients + dealing with Long-Term Conditions & Diabetes Focus

About the Role
We’re looking for a dedicated Practice Nurse with experience in managing long-term conditions to deliver high-quality remote patient care.
You’ll work as part of a supportive, multidisciplinary team providing telephone and video consultations to patients, supported by excellent clinical systems and digital infrastructure.
Key Responsibilities
* Conduct remote reviews and consultations for diabetes, hypertension, asthma, and COPD patients.
* Support patients in managing their long-term conditions through structured care plans and education.
* Accurately document consultations and outcomes on SystmOne.
* Work collaboratively with GPs, pharmacists, and other clinicians to ensure continuity of care.
What We’re Looking For
* Registered Practice Nurse (NMC Pin)
* Strong background in chronic disease management (particularly diabetes)
* SystmOne experience is essential
* Confident conducting remote consultations
* Excellent communication and patient engagement skills
