Responsibilities
* Participate in the delivery of countywide specialist learning disability nursing with a focus on reducing health inequalities under the guidance of the Lead Nurse.
* Provide professional supervision to other staff and students, including appraisal and professional development, particularly mandatory training.
* Contribute to the planning, development, and evaluation of learning disability services and to defined projects delegated by the LDP management team via the Lead Nurse.
* Act as an autonomous practitioner managing a specialist caseload providing assessment, identification, and treatment of the health needs of adults with a learning disability.
* Participate in multidisciplinary/multi‑agency working within the LDP and with key partners.
* Focus on health facilitation, health promotion and linking with primary, secondary health and neighbourhood teams to reduce health inequalities.
Requirements
* Full UK driving licence and use of a personal vehicle; public transport cannot be used for this role.
* Ability to travel independently around the county meeting strict time deadlines.
* Experience in managing specialist caseloads and working in a multidisciplinary environment.
* Professional supervision and appraisal skills.
