Job summary

Work in collaboration with Bevan colleagues todeliver a coordinated and high-quality service that proactively encourages serviceusers to access and engage with community support. To work as part of theHealth Inclusion Outreach Team through attendance at Shelters, Soup Kitchens,Volunteer/Charity Sector locations, HMO accommodation and more.

Main duties of the job

* Manage safeguardingconcerns appropriate to your role and level of training.
* To plan, develop andfacilitate group and 1-2-1 sessions.
* Offer comprehensive support to empower people totake control of their health and wellbeing. You will provide social, emotional,and practical support tailored to the service users needs and facilitateaccess to community services, pathways to recovery, housing solutions, socialreintegration, and mental health support.The role is not intended to support individuals long term, but tohelp them to understand how they can support themselves better by exploringbarriers to improved mental, social, and physical health.
* Contribute to the development of the service andparticipate in support, supervision and training as required.
* Contribute to theeducation of staff within the network and maintain details and grow workingknowledge of sources of support in the community.

Job responsibilities

* Develop trustingrelationships with service users, giving them time to focus on their assets andwhat matters to them and providing them with personalised help to takecontrol of their health and wellbeing, live independently and better understandthe impacts of their lifestyle choices.
* Undertake holistic clientneeds assessments using a wellbeing template. Support, training and supervisionwill be given.
* Support a caseload ofclients for whom wellbeing support and mentorship might offer improvedoutcomes. With the team, proactively identify people who would benefit fromthis type of help and manage and regularly review your caseload to accommodateurgent referrals for support as required, referringout to community support services as soon aspossible.
* With clinicalsupport, develop a strong awareness and understanding of when it is appropriateor necessary to escalate care back to the GP or other care professionals whenthe persons needs are beyond the scope of the health and wellbeing supportworker e.g. when there is a mental health need requiring a qualifiedpractitioner.
* Understand whenappropriate to undertake clinical skills and assessments on clients and provideclinical skills outside of a clinical setting. I.E Phlebotomy, baselineobservation, spirometry.
* Provide clinician ledcare, under direct and indirect supervision of a clinician. I.e wound care,vaccinations etc.
* To be pro-active inmaintaining personal and professional learning and development and identify owntraining needs as they arise.
* To attend teammeetings, away days and other similar events.
* To participate fullyin regular one-to-one supervision sessions and in an annual performanceappraisal.
* Work as part of theteam to seek feedback, continually improve the service, and contribute tobusiness planning.
* Undertake any tasksconsistent with the level of the post and the scope of the role, ensuring thatwork is delivered in a timely and effective manner.
* Ensure compliancewith all relevant policies and procedures in relation to health and safety,equal opportunities, safeguarding, lone working, customer service, dataprotection and confidentiality.

* Experience of working with health inclusion groups including Asylum Seekers and Refugees, people experiencing homelessness and the Gypsy, Traveller, and Roma communities.
* Experience of assessing need and working within time-limited interventions to promote and support change.
* Experience of planning, facilitating, and evaluating workshops / groups, focused on self-help.
Special Knowledge & Skills

* Knowledge of local support agencies and a commitment to the continual updating of personal knowledge in this area.
* Excellent communication skills, both verbally and in writing.
* IT literate, with a sound working knowledge of Microsoft packages and data management/case recording systems.

* The ability to speak an Eastern European, African, or Middle Eastern language.
* Knowledge of how to work within the biopsychosocial framework.
