Overview
We have an exciting opportunity for a hardworking and enthusiastic individual to join our Assessment Management Team as an Occupational Therapy Assistant within Blantyre LIFE Intermediate Care.
Blantyre LIFE Intermediate Care is a 24-hour, short-stay service delivered by South Lanarkshire University Health and Social Care Service. The service is designed to reflect a home environment and operates using a reablement model of care. All service users have experienced a hospital admission and are medically fit for discharge but require additional support before safely returning home.
Responsibilities
As part of the Assessment Management Team, your role will focus on supporting service users to regain independence through Occupational Therapy principles and practice, enabling them to return home as quickly and safely as possible. You will work closely with individuals within the Blantyre LIFE Intermediate Care Unit, undertaking home visits, and facilitate home discharges.
* Assessment, practice, and evaluation of functional abilities
* Supporting improvements in independence and confidence
* Identifying and recommending appropriate equipment where required
* Making onward referrals to statutory or voluntary agencies to meet identified needs
Qualifications and Skills
* You should be able to work autonomously and meet tight timescales.
* You should be able to demonstrate responsibility for your own learning needs.
* You should have an HNC in Occupational Therapy Support or be willing to work towards gaining this qualification.
