Health & Justice RECONNECT Band 6 Practitioner
Midlands Partnership NHS Foundation Trust is recruiting a Reconnect Practitioner to work within the Health and Justice Reconnect Team in Staffordshire.

The role involves working both within prison and the community. Prisons in the Staffordshire area include: HMP Brinsford, HMP Dovegate, HMP Drake Hall, HMP Featherstone, HMP Oakwood, HMP Stafford, HMP/YOI Swinfen Hall, and YOI Werrington.
The Reconnect service identifies individuals with substance misuse, mental health, learning disability or other vulnerabilities who encounter the criminal justice system. The team assesses unmet needs, offers referral and sign‑posting, and liaises with agencies to support onward planning and intervention.
Assessments are provided to individuals prior to release and ongoing support is offered up to 28 days post‑release. Brief community‑based intervention is delivered to maximise service‑user access and engagement with appropriate services for up to six months, or until health needs are met.
All‑age service: the post requires working with individuals aged 15 and above.
Subject to prison vetting and expected to travel across the MPFT footprint.
Key Responsibilities
* Conduct holistic health assessments in collaboration with service users and carers, promoting self‑determination, choice and autonomy.
* Identify individual needs, agree and refer to appropriate agencies.
* Contribute to robust risk assessments and risk management plans, promoting positive risk‑taking where appropriate.
* Maintain effective reporting through observation and electronic health record entry.
* Review effectiveness of care using agreed outcome measures.
* Use Silvercloud and Justice Star tools in supporting service users.
Qualifications
* Professional recognised qualification such as RMN, RGN, Learning Disability Nurse, Social Worker, Occupational Therapist, or AHP.
* Degree qualification in a relevant subject or demonstrable evidence of equivalent experience.
Knowledge, Skills & Abilities
* Knowledge of mental health issues, learning disability, and the criminal justice system.
* Understanding of health screening tools, risk assessment and mitigation, including risk of harm to self and others.
* Awareness of vulnerabilities, including the needs of children and young people, capacity and consent, and theoretical approaches to working with vulnerable people.
* Knowledge of relevant legislation: Mental Health Act, Mental Capacity Act, Children Act, Social Care Act, PACE Code C, safeguarding, information governance, GDPR.
* Specialist knowledge in substance misuse, autism, ADHD, personality disorder, and domestic violence.
* Knowledge of psychosocial and psychological approaches.
