Are you passionate about supporting people with mental health presentations to live meaningful and fulfilling lives in their community?

Devon Partnership NHS Trust is seeking a committed and compassionate Community Support Worker to join our dedicated Baywide team based at the Chadwell Centre in Torbay.

Under the daily supervision of qualified mental health practitioners, you will play a key role in a multi-disciplinary team, working collaboratively to deliver person-centred, recovery-focused care for individuals experiencing a range of mental health difficulties in the community.

As a Community Support Worker, you will:

Work closely with clients in their own homes and community settings to support recovery and independence.

Help prevent hospital admission and assist in safe, timely discharges back into the community.

Engage across agencies, including Primary Care, Complex Care Teams, and Third Sector organisations, to coordinate effective care.

Implement and monitor individual recovery care plans under the guidance of mental health practitioners.

Undertake a significant monitoring role, providing valuable feedback and updates to the wider clinical team.

Main duties of the job

* To communicate effectively with people using services takinginto account any communication difficulties they may have and respondingappropriately.
* To participate in,support and maintain positive interventions and planned activity programmes toimprove the quality of life and mental state of the person using services.
* The post holder will be aware of and work within their ownlimits, and competencies.
* Take part in theconstant assessment of risk and management of risk in conjunction with thequalified staff.
* To liaise appropriately with other Agencies as part ofsupporting the delivery of care.
* To independently or jointly monitor a persons mental healthto ensure safety. This may require intensive input and /or regular monitoringas directed by the mental health practitioners within the team and inaccordance with the recovery care plan.

Person Specification


Knowledge & Skills

* Awareness of safeguarding principles
* Demonstrate an ability to make and sustain professional therapeutic relationships
* Knowledge of mental health and a basic understanding of physical health presentations.


Qualifications

* Basic Education
* To complete the Care Certificate within the first 6 months of employment unless already obtained or evidence of equivalent agreed and signed off by the appointing manager at interview. If not, this will be a trainee position until successful completion
* NVQ level 3, QCF Level 3 in Health and Social Care or equivalent qualification.
* Other evidence of training or development relevant to working within mental health settings.


Experience

* Knowledge of mental health and a basic understanding of physical health presentations
* Previous care experience with people who have Mental Health difficulties.


Physical Skills

* Do you have the ability to travel to meet the requirements of the role?
