Hospice Community Clinical Nurse Specialist
StElizabeth Hospice continues to drive forward innovation and development ofservices, both in the Ipswich and East Suffolk locality and further up thecoast to the Great Yarmouth and Waveney area.In 2019 NHS Norfolk and Waveney CCG awarded the specialist palliativeand end of life care contract to East Coast Community Healthcare. Thecontract includes a specialist palliative care service which will be deliveredin partnership with St Elizabeth Hospice.Our most recent Care Quality Commission report rated the hospiceas Outstanding.
InMarch 2023, St Elizabeth Hospice merged with East Coast Hospice to form StElizabeth East Coast Hospice, with all service provision in Great Yarmouth andWaveney coming under the St Elizabeth East Coast Hospice name.
Thisis an exciting time for St Elizabeth East Coast Hospice, as we are developingan achievable and sustainable palliative and end of life care service for theresidents of Great Yarmouth and Waveney. Its a time of expansion, developmentand investment in the Hospice Team. We areoffering an opportunity to be part of the Community Specialist Palliative CareTeam and also work within our Community Care Unit influencing the holistic careof patients and carers with end of life and specialist palliative care needs. Main duties of the job
Are you an enthusiastic Registered Nurse with demonstrable experience and knowledge of providing care to people with a specialist palliative care need?
We are looking for an experienced Registered Nurse to join our Community Team. Youwill work with our multi-disciplinary team providing specialist palliative care advice and support to patients in the Great Yarmouth and Waveney area in the community and at one of our Community Care Units.

JOBSUMMARY
To providespecialist palliative nursing care to patients and their families within aservice that works every day, by phone, by visits and advising and teachingothers within the community and at our community unit locations.
CLINICALRESPONSIBILITIES
To work within a teamproviding specialist palliative care to patients, within a community setting, communitycare units and care homes.
Supporting carers andother providers delivering care, through the advice line, training andeducation.
To undertake in-depthholistic assessments of patients with palliative care needs, regardless oftheir diagnosis, and plan care to meet patient outcomes. To provideinformation, advice, psychological and spiritual care to patients, theirfamilies and carers, liaising and referring on as necessary to appropriatehealthcare professionals in the Hospice, hospital and community.
To visit patients andfamilies when a specialist consultation is needed and the patient is unable totravel, or seeing patients in the community clinic.
To provide telephonicsupport to patients, families and other professionals provided during workinghours.
To attend andparticipate in multi-disciplinary palliative care meetings around patient caremanagement, and present patients at the community MDT and to the caseloadreviewer.
Provide a rapidresponse service for patients needing urgent specialist care. Providing care asneeded such as drugs, position changes and interventions, if appropriate toimprove the comfort of the patient, or advising others to do so.
To represent the Hospiceand patients at meetings, as required.
To maintain and promoteeffective and efficient working and communication with all involved in thepatient, to ensure appropriate sharing of information.
To provide support and expertise to other professionals,as required, acting as a role model and a resource providing expert knowledgeand support.
Tosupervise junior roles to be responsible for supervision, annual appraisalsand delegation of workload.
To comply with relevant drug policies, includingNon-Medical Prescribing and legislation concerning drugs and administration.
To attend Gold Standard Framework (GSF) meetings.
