3-month role with a local authority
Summary
* This is a 3-month role with a local authority focused on addressing and reducing rough sleeping. The service collaborates with internal and external partners to identify and respond to reports of rough sleepers, maintaining an accurate overview of the local situation. The role involves providing advice and referrals to services that meet individual needs, such as healthcare, substance misuse services, housing, and benefits. The ultimate goal is to help individuals address their needs, improve their health, and find solutions to end their homelessness, encouraging a more settled lifestyle through support and appropriate referrals.
Responsibilities
* Build and maintain productive relationships with internal and external stakeholders, including local authorities, homeless services, and housing providers.
* Offer a client-centered approach, providing support and advice to access healthcare, work and training opportunities, and other necessary support services.
* Develop client-centered support plans for those in medium or long-term accommodation to prevent a return to the streets.
* Collaborate closely with the Outreach team, Housing demand team, and Temporary accommodation team to identify clients at risk of homelessness.
* Visit clients at their temporary accommodation or sleeping site to provide tailored support aimed at preventing homelessness.
Requirements
* Demonstrable extensive experience in delivering high-quality outreach services for rough sleepers and those in temporary accommodation.
* Essential qualification or equivalent experience in a similar role.
Additional Information
* Working hours: 37 hours per week
* The role requires a DBS check.
* This is not a hybrid role; the candidate will be required on-site Monday to Friday, 9 am to 5 pm. There is also a work rota for outreach sessions early morning (5 am - 1 pm) or late at night (5 pm to 1 am).
