Community Support Worker - IAT Service

Main duties of the job
Build and maintain effective working relationships with community teams, healthcare professionals, inpatient and crisis services, GPs, Adult Social Care and third‑sector organisations. Establish positive, supportive relationships with service users, their relatives and carers, ensuring clear and effective communication. Provide information and practical support to help service users and carers understand their rights, entitlements and care plans. Monitor mental health needs through observation and engagement, reporting changes to keyworkers to ensure care and risk plans remain up to date. Support service users with daily living needs, including housing‑related support, transport (in line with care plans and Trust guidance), and basic physical health needs such as diet, hygiene and sleep. Encourage engagement in social, physical and recovery‑focused activities that support independence and wellbeing. Assist with the development of Crisis and Contingency Plans and co‑facilitate group activities under the direction of qualified staff.
This is a community‑based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.

This role is currently open only to people who already work for the NHS in Cornwall or the Isles of Scilly. Only current employees of the following organisations can apply:

Person Specification
Education / Qualifications and Relevant Experience
* NVQ3 in healthcare or equivalent qualification or experience.
* Experience working with clients with a range of mental health problems.
* Understanding of CBT methodology.
* Basic understanding of current National Guidelines and Mental Health Act / Mental Health Capacity Act requirements.
Knowledge and abilities
* Knowledge of the effects of mental health or learning disability issues on service users.
