Key Responsibilities
* Support the District Nursing team through the management of an assigned caseload.
* Assess, plan and evaluate the patient care needs.
* Work closely with the District Nurse to facilitate effective communication regarding patient care.
* Maintain up‑to‑date care plans and ensure seamless continuity of services, especially during reviews of complex care arrangements.
* Contribute to mentoring, educational and assessment activities.
* Collaborate with healthcare professionals and external agencies to provide efficient and high‑quality service delivery.
