We are looking for an individual who would like to be part of a dynamic and forward-thinking team. Do you enjoy the challenge of a wide variety of work? Could this be you?

The role will include visiting patients at home to assess for, and provide, therapy interventions in collaboration with the registered therapy colleagues within the team. The focus of the role is to support admission avoidance, to promote independence and to assist in the effective management of patients with long term conditions.

The post would suit someone interested in working within an integrated therapy team and wanting to achieve the best for their patients. They will work within a wider multi-professional team including Community Nurses and Advanced Clinical Practitioners to provide individualised patient care.

Only open to applicants from Hampshire & Isle of Wight NHS system, which is limited to: Hampshire and Isle of Wight ICB, Hampshire & Isle of Wight Healthcare FT, Portsmouth Hospitals University NHS Trust, University Hospital Southampton FT, South Central Ambulance Service, Isle of Wight NHS FT or Hampshire Hospitals FT.


Main duties of the job

Work as part of the multi-disciplinary integrated Community Independence Service to undertake assessment, treatment and monitoring of planned care for patients who require rehabilitation in the community, seeking guidance from the registered professional as required.

Provide support and health promotion advice under the guidance of registered professionals.

May be required to supervise junior staff on a day-to-day basis.

Qualifications


Foundation Degree in Health and Social Care

* Foundation Degree in Health and Social Care
* Further training and completion of competencies working across OT, Physio and Nursing will be required within twelve months of being in post if not already attained.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
