Assistant Practitioner/Supported Assistant Practitioner roles 8749 | Permanent contracts | £14.82 to £17.46 per hour (Scale F/G) | Full/part time hours considered | County Hall, Norwich
Responsibilities
* Support individuals and communities within Norfolk’s Living Well approach by encouraging meaningful conversations with people to create better outcomes.
* Strengthen the front door of social care by reducing process-driven hand-offs and promoting connection with local networks and community support.
* Maintain excellent communication, accurate record-keeping, and keyboard skills to document care and interactions.
* Work in a multidisciplinary team, participate in regular supervision and training, and contribute to learning and development.
* Operate in a hybrid working arrangement (home and office) with flexibility to meet service demands.
Qualifications
* Excellent communication and record-keeping skills; proficient with keyboard/input tasks.
* Experience working with individuals who require care and support.
* Commitment to excellent customer service and ability to manage a high workload in an organized and efficient manner.
* Strong commitment to multidisciplinary working and familiarity with the department’s eligibility criteria.
