Community Physiotherapist

We are looking for an enthusiastic Band 6 Physiotherapist to join our Intermediate Care Rehabilitation Therapy Team based at Blandford Hospital.
The role is to provide Physiotherapy treatment to patients with acute illness to prevent unnecessary hospital admission, facilitate complex discharges from hospitals and support patients with long and short‑term conditions. Predominantly the work is in the community, although ward visits to the local Community Hospital may be required. A valid UK driving licence and access to a car are essential.
As a Band 6 Physiotherapist you will have the opportunity to:

Main duties of the job
Provide Physiotherapy assessment and treatment to people in their own homes, care homes, or on Community Hospital wards, depending on locality.
* Deliver high‑quality person‑centred care that prioritises safety, privacy and dignity.
* Provide specialist care and interventions, assessments, treatments and caseload management as part of a multidisciplinary team.
* Offer specialist advice to colleagues regarding patient management and service delivery.
* Develop and ensure delivery of specialist programmes of care for other professionals.

Person Specification
Knowledge, skills and training
* Registered practitioner to degree/diploma level supplemented by post‑registration diploma level specialist training and/or short courses or extensive experience in the relevant specialty.
* Membership of the relevant professional body.
* Evidence of recent professional development in an up‑to‑date portfolio.
* Current valid UK driving licence.
* Learning and Assessment in Practice qualification or equivalent practice assessor training.
* Non‑medical prescriber (where applicable).
* Knowledge and understanding of Trust strategy relevant to the role.
Job Specific Experience
* Experience at Practitioner Band 5 level.
* Demonstrable knowledge of assessment and therapeutic interventions in the area of specialism.
* Ability to demonstrate specialist clinical reasoning skills to assimilate information for clinical judgment regarding diagnosis and intervention.
* Ability to prioritise and organise workload effectively.
* Recent previous experience within a comparable role.
* Experience of managing change.
Managerial/Supervisory Experience
* Experience providing clinical supervision and mentoring to junior staff.
* Experience devising and delivering training.
Personal Qualities / Attributes
* Experience planning and organising complex activities, including discharge planning and coordination with other agencies.
* Knowledge and understanding of legislation relevant to practice.
* Ability to use tact and diplomacy.
* Analytical thinking with the capacity to provide resolutions to situations.
* Demonstrable leadership qualities and the ability to act as a role model.
