We are seeking a qualified, compassionate, and motivated Mental Health Practitioner with experience in acute mental health settings. You will hold a professional registration (NMC, HCPC, or equivalent), demonstrate strong assessment and risk management skills, and show a commitment to delivering high-quality, person-centred care.
Resilience, adaptability, and the ability to work effectively within a multidisciplinary team are essential, as you will be supporting service users in crisis. You will provide intensive and responsive crisis resolution and home treatment to adults aged 18 and over, helping individuals remain in the community and avoid unnecessary hospital admissions.
The role is based within Pennine Care NHS Foundation Trust, with flexible working across a 24/7 shift pattern to ensure continuous service provision. You will be part of a skilled multidisciplinary team, collaborating with community mental health services, acute mental health services, VCSE partners, and psychological professions.

Main duties of the job
· Prioritise and respond to service users referred to the CRHTT, managing safety and response times.
· Provide intensive crisis resolution and home treatment, supporting gatekeeping of admissions and facilitating early discharge.
· Conduct psychosocial and risk assessments, develop care and safety plans, and deliver specialist interventions.
· Lead and coordinate daily team activities, manage referrals, and facilitate timely discharges.
· Provide clinical supervision and support to junior staff and students.
· Maintain accurate clinical records and ensure effective communication within the team and with partner agencies.
· Adhere to professional, clinical, and social care governance standards, and participate in service development and audit.

Education/Qualification
Essential criteria
* Qualified and Registered Mental Health Nurse, Occupational Therapist or Social Worker
* Current registration with professional body plus experience and short courses to post graduate diploma level
Desirable criteria
* Non-medical prescribing qualification
* Further training in brief psychosocial interventions.
Experience
Essential criteria
* Significant post registration experience
* Proven experience of working with clients with complex needs
* Experience of working with individuals who present to mental health services in crisis and have symptoms of mental health illness
* Experience of providing supervision
Desirable criteria
* Adult acute mental health work experience
* Experienced in the use of outcome measures such as CORE, PHQ9 and GAD
* Experience of working with a recovery model of care
Knowledge
Essential criteria
* Understanding of current mental health legislation, Safe Guarding and Child Protection Legislation
* Knowledge of psychosocial assessment and risk assessment / management processes
* Awareness of NICE Guidance in the treatment of mental health problems and knowledge of treatments and interventions appropriate for individuals with mental health needs
Skills and Abililties

* Ability to undertake comprehensive mental health and risk assessments
* Ability to construct effective packages of care and risk management plans
