This post is fixed term/secondment until 5 January 2027 due to needs of the service

The postholder will work closely, in a collaborative way with colleagues across acute and secondary care providers to develop ways of identifying and responding effectively to meeting the needs of people with Learning Disabilties who are admitted to services.

The post holder will be creative and dynamic in realtion to promoting and championing the rights of people with learning disabilities to access healthcare.

The postholder will support colleagues in acute and secondary care to assess people with learning disabilities referred to the service, and will provide advice and guidance to hospital staff regarding:-

• Making reasonable adjustments within the hospital environment.

• Managing behaviour which may challenge services.

• Additional complex health needs experienced by individuals.

The postholder will act as a resource for hospital staff and will provide education and training to raise the profile of people with Learning Disabliites and ensure that their health needs are met.

Key Tasks and Responsibilities of the Post: The role is 50% clinical and 50% educational.

Main duties of the job

Develop links between the Liaison Service and all clinical areas within Acute and Secondary health provision in Health Board.

• Work in collaboration with Acute/Secondary Care; Primary Care; CLDT’s; AATU’s and other key stakeholders from health, social care and other agencies.

• Contribute to the development of care pathways to increase awareness of and reduce impact of health inequalities experienced by people with Learning Disabilities.

• Contribute to the identification of barriers to access to healthcare and develop action plans to address.

• Frequently receive and communicate highly complex and sensitive information to/about people with Learning Disabilities, their families, carers and other professionals, to ensure the needs of individuals are met.

• Provide guidance, support and education to people with Learning Disabilities, their families and carers accessing healthcare when required.

Qualifications & Knowledge

Essential criteria

1. R.N.L.D (Learning disability nurse)
2. NMC Registrant
3. Educated to degree level
4. Relevant post-graduate studies.
5. Evidence of commitment to ongoing professional and personal development

Experience

Essential criteria

6. Understanding of the mutli-agency input into Learning Disabilities Services, and experience of working in a Community Team
7. Knowledge and understanding of the additional health needs of People with Learning Disabilites; Mental Capacity Act; reasonable adjustments to ensure access to healthcare
8. Knowledge of the role and function of Acute/Secondary Care settings

Aptitude and abilities

Essential criteria

9. Adhere to and can demonstrate SBU Values & Behaviours
10. Ability to build and promote effective working relationships with a diverse range of professionals, agencies, and relatives
11. Ability to analyse and communicate complex data and information to support decision making processes
12. Established communication, negotiation and influencing skills; verbal and written. Proven ability to motivate and influence others
13. Proven ability to implement plans of care in relation to people with Learning Disabilities who have additional health needs

Desirable criteria

14. Welsh Language Skills are desirable levels 1 to 5 in understanding, speaking, reading, and writing in Welsh (please delete as appropriate)
