Responsibilities
* Risk assessment and care planning around individual client needs.
* Supervision and appraisal of staff.
* Communicating effectively with clients, their families, staff teams and colleagues in the multi-disciplinary team.
* Rota management.
* Manage packages of care for those with Mental Health needs and acquired brain injuries as a Field Care Manager within the team.
* Monitor support packages including reviewing support worker documentation.
Required Skills and Qualifications
* A passion for good quality care.
* Self-motivation and personal drive; team player with respect for other team members.
* Good communication skills with internal and external customers; customer-focused.
* Can-do attitude; able to work on own initiative.
* Able to adapt to changing working practices and environment.
All staff will be subject to satisfactory DBS and reference checks.

* Salary Range £25,000 - £27,000 p.a. (inclusive of on-call responsibilities).
