As a Registered Nurse (Band 5), you'll be an integral part of our Primary Care and clinical DART team, supporting some of our most vulnerable patients and planning care for people with complex and challenging health needs. You'll be confident in all aspects of primary care delivery - including undertaking health assessments and performing diagnostic tests - and able to work flexibly to meet the needs of our service.
We're looking for a candidate with lots of energy and an enthusiastic, "can do" attitude. As part of a multidisciplinary team, you'll be working closely with Mental Health and Substance Misuse services to optimise on-site care and you'll need to form working relationships with a wide range of partners.

Main duties of the job
1. Working closely with our management team and key stakeholders, you will contribute to the delivery of a comprehensive primary care health service.
2. You will be part of a multi-disciplinary team, undertaking physical health assessments of patients, planning care, and providing a range of health treatments/interventions.
3. You will support patients to address issues that affect their own health, behavior, and social welfare.
4. Manage competing priorities and react to unforeseen circumstances, incidents, and emergency situations.
5. This role includes rotation to weekends and night shifts.

Person Specification
Qualifications (Essential): Registered Nurse Level 1 - Adult / General / Mental Health
Qualifications (Desirable): Mentorship / supervision / teaching / assessing experience / training
Knowledge and Experience (Essential): Knowledge of current healthcare issues, clinical governance, risk management, working within a multidisciplinary team, clinical skills such as phlebotomy and ECG, experience managing deteriorating patients, understanding confidentiality.
Knowledge and Experience (Desirable): Experience in Health & Justice, health needs assessment/Triage, general practice, Minor Injuries, and familiarity with SystmOne.
