We are seeking an experienced and dynamic Critical Care Outreach Nurse to join our multidisciplinary team. This is an exciting opportunity to work across two acute hospital sites, delivering high-quality care to deteriorating patients and supporting clinical teams in the early recognition and management of critical illness. This is a 24-hour, 7-day service, requiring flexibility to work across both hospital sites. Shift patterns will include days, nights, and weekends.

Main duties of the job
You will play a key role in:
1. Acute Kidney Injury (AKI) Management: Providing specialist input for patients with AKI, supporting early intervention and prevention strategies.
2. Stabilisation and Transfer Service: Leading and assisting in the safe stabilisation and transfer of critically ill patients between wards, departments and hospital sites.
3. Critical Care Outreach: Responding to referrals, supporting ward teams with patient assessment, a core member of the cardiac arrest team, following up patients discharged from the critical care unit, responding to calls from relatives and patients in line with Martha's law and delivering education to improve patient outcomes.

Is responsible for the provision of Excellent Care with Compassion
Provides education using a variety of methods and opportunities to support the delivery of care with compassion for the deteriorating or acutely ill patient with additional complex needs.
Supports the development of an evidenced based critical care outreach service, utilising national recommendations, research, and evaluation of current clinical practice. To ensure the on-going delivery of high standards for patients at risk of deterioration.
Communicates in a well-structured clear professional manner at all levels, using a variety of methods.
Takes personal responsibility for own professional competence and registration and supports team working.
Is responsible for the monitoring and provision of trust values and standards that create a safe, clean and organised environment/resource to manage patients reviewed and supported by the critical care outreach team
Safeguards people and promotes equality in care and practice by recognising, respecting and meeting the needs and choices of individuals
Person specification
Qualifications & Education
Essential criteria
4. Appropriate NMC Registration or other professional body
5. Evidence of experience and qualification in teaching (may be a module as part of registration or post registration short course or proven ability).
6. Post Registration Qualifications in Critical care or High Dependency Skills course or equivalent course / experience relevant to the role
7. Diploma or Degree in Nursing/Healthcare research
8. Evidence of Education to Degree Level.
9. Evidence to support Leadership and Development within own practice
10. Acute illness management course or similar.
Desirable criteria
11. Level 4 ILM (Institute of Leadership and Management) or equivalent
12. Mentorship qualification
13. Level 6 or 7 Non-medical prescribe
14. Working towards academic study at level 7
15. Clinical assessment and physiological examination qualification at level 6 or 7
Knowledge & Experience
Essential criteria
16. Able to perform clinical duties commensurate to that of the role e.g. IV medications, Cannulation, supporting tracheal intubation
17. Ability to Implement changes within nursing practice
18. Knowledge of Clinical Governance and Safeguarding and the relevance of these to practice
19. Knowledge of relevant policies and procedures
20. Knowledge of ethical issues
21. Advanced level of Critical or Acute Care Experience
22. Demonstrates a sound working knowledge of current issues within the NHS especially developments within national critical care service strategies, including critical care outreach.
23. Proven experience/ability to manage manpower and other resources
24. Able to demonstrate a personal contribution to the continuing professional development of staff
25. Able to demonstrate a flexible approach to managing staff and physical resources
26. Able to lead a team and co-ordinate the work of others
27. Knowledge of Ethical and Legal issues within healthcare
28. Willingness to undertake relevant further study as required for the post

29. Experience of managing an acute clinical area
30. Knowledge of the appraisal process
31. Evidence of working at a senior level
