Salary: Starting salary from £24,500 per annum dependant on experience

Working hours: 37.5 hours per week

Mid Chiltern Primary Care Network have an exciting opportunity for a Pharmacy Care Coordinator to work within our dynamic Clinical Pharmacist team.

An exciting opportunity has arisen for a Pharmacy Care Coordinator to join our already established, growing, and committed PCN team in providing additional care and services across our patient population.

Mid Chiltern Practices:

· Rectory Meadow Surgery

· Amersham Health Centre

· John Hampden Surgery

· Hughenden Valley Surgery

· Prospect House Surgery

The successful candidate will possess strong organisational, administrative and communication skills with the ability to multi-task. You will need to demonstrate that you have a compassionate nature with drive for achieving high quality, safe care. You must be a conscientious, self-motivated individual with a methodical manner and the ability to work well on their own as well as part of a multi-disciplinary team.

The successful candidate will be joining a thriving existing PCN team of pharmacy technicians and clinical pharmacists and be an integral part of the team when it comes to delivering the best patient care.

Primary Duties and Areas of Responsibility

Supporting the clinical pharmacists and pharmacy technicians by:

* Booking patients into various pharmacy clinics, such as BP, asthma, diabetes, medication monitoring, medication reviews
* Dealing with tasks that come into the shared mailbox
* Booking patients in for their COVID and flu vaccinations
* Managing clinics throughout the year for the team

Support Learning disability patients, maintaining contact and inviting them in for their annual review

Manage cancer review recalls and deal with invites for reviews for:

· 3-month discussions and 12-month reviews

· Bowel screening DNA

· Breast screening DNA

New diagnosed patients
