Role Purpose The Social & Community Care Worker plays a vital role in supporting adults to maintain independence, wellbeing, and dignity within their own homes and local communities. This role involves delivering person-centred care, assisting with daily living tasks, and promoting social inclusion through meaningful engagement. Key Responsibilities 1. Person-Centred Care & Daily Living Support Provide personalised support with daily tasks. Assist with routines, personal care and mobility. Promote independence and autonomy. 2. Community Access & Social Inclusion Support access to appointments, groups, and activities. Reduce social isolation through meaningful engagement. 3. Emotional Support Build positive, trusting relationships. Promote emotional wellbeing, reassurance and stability. 4. Safeguarding, Risk & Safety Identify and report safeguarding concerns. Follow risk assessments and maintain safe environments. 5. Documentation & Record Keeping Maintain accurate notes and logs. Ensure compliance with GDPR and confidentiality. Person Specification Essential: Community care experience. Understanding of safeguarding. Strong communication skills. Desirable: NVQ Level 2/3. Experience supporting LD, MH or older adults.
