Making a Difference as an Assistant Team Manager
We are seeking a driven and innovative qualified Assistant Team Manager / Social Worker to join the Integrated Health and Social Care within Contact Cares, Front Door Service. This large, busy multidisciplinary team includes senior social workers, nursing and therapy professionals, and both qualified and unqualified health and social care staff, focusing on early intervention to improve lives, promote independence, and prevent hospital admissions.
The team conducts assessments, care planning, and handles initial and onward referrals and signposting for a wide range of adult clients across health and social care. The team operates from 8:00 am to 10:00 pm, 7 days a week including bank holidays. This position is part-time, at 18.5 hours per week, within an MDT care setting.
