This is an exciting opportunity to play a pivotal role in the continued development of our expanding neonatal outreach service. The post holder will lead on both project delivery and people management, working collaboratively within the wider perinatal team to drive high-quality, family-centred care.

A key focus of the role will be the full development of Transitional Care, ensuring alignment with MIS safety actions and supporting our ambition to keep families together whenever possible. This position will be instrumental in shaping and delivering these improvements.

Our Neonatal Unit is one of the busiest Local Neonatal Units (LNU) within the Southwest Neonatal Network. We are committed to expanding our outreach service to a fully established seven-day model, and this role will provide the leadership required to achieve that vision.

Additional working pattern details: Weekends, Bank holidays, seven days/week

An experienced senior neonatal nurse responsible for operationally leading and maintaining clinical leadership and management in the care of neonates and their families. To provide expert support and advice relating to the provision of Transitional care and the Neonatal Outreach Service.

To operationally lead and manage the neonatal nursing team alongside other members of the Senior Neonatal Leadership Team. You'll be an expert leader in neonatal care, with specialty qualification, able to deliver neonatal clinical care across a range of ITU/HDU/SCBU and Transitional Care Cots. Proven ability to effectively manage and professionally lead the clinical nursing team and engage with service users and families.

Project management, audit and Quality Improvement skills will be essential asset to the post holder.

This post will involve some visiting of patients in their homes, therefore car driver is essential

* Takes the operational responsibility for provision of effective, quality care within Transitional care and the Neonatal Outreach Service following a family integrated model of care.
* To work alongside the Matron, the ward managers and wider teams to determine and deliver on service strategic objectives, encouraging interdisciplinary working.
* Manages a team of Neonatal Nurses and Neonatal Nursery Nurses creating constant communication link between Medical, Midwifery and Nursing staff on the Neonatal unit, Transitional Care, Delivery Suite and the Community Neonatal Outreach Service.
* Participates in Network Neonatal service development.
* Provides senior level contributions to resource management in the provision of neonatal services.
* Project manage the implementation of increasing this service
* Ability to run outpatient clinics
* Ability to support home oxygen therapy
* Engages in Clinical Governance initiatives and leads on risks associated with Neonatal Outreach and Transitional Care.
* Leads the discharge process for infants requiring Neonatal Outreach care
* Develops effective links with the Trust Quality Improvement team to enable the support of staff undertaking QI Projects.
* Represents Neonatal care at appropriate local, network, and national forums.
* Leads on the implementation and evaluation of care pathways within T.C and the Neonatal Outreach Service.
* Promotes a progressive attitude to the continual improvement of care through research, evidence-based practice and a knowledge and understanding of up to date National Neonatal and perinatal drivers.
