The Adult Community Nursing service Single Point of Access (SPA) team is looking for a Nurse Practitioner to join its Clinical Assessment Team (CAT). This post is ideal for nurses who are enthusiastic, responsible and committed to the highest standard of care.
The CAT Team is situated within the Adult Community Nursing Service at Homerton Healthcare and works alongside District Nursing Neighbourhood Teams and Specialist Services including Tissue Viability and Continence nursing teams. The Team works across City and Hackney and undertakes holistic care assessments and implementation of care in patients' homes, and hospital admission avoidance through assessment and care of acutely unwell patients at home.
The successful candidate will work closely with the Senior Nurse Practitioner and the multidisciplinary team and will possess excellent communication skills. They will be able to utilise clinical knowledge and evidence-based practice to independently undertake care and assessment in patients' homes. They will have an understanding and experience of lone working and work within their own limitations knowing how, when and to whom to escalate concerns. They will be motivated, compassionate, well organised, and committed to developing clinical skills and knowledge.
Main duties of the job
The post holder will use their clinical skills and judgement to promote independence and support individuals to self-care; deliver short-term nursing care interventions, sign post to other neighbourhood services and transfer to the District Nursing teams if they require longer-term care. They will work in the Clinical Assessment Team, undertaking initial holistic assessments on individuals referred to the service and implementing care plans.
They will also demonstrate professional leadership and support junior colleagues to develop their clinical knowledge and skills. The post holder will practice within a multidisciplinary team and provide care to a group of patients, assessing, planning, implementing and evaluating their care needs. They will be responsible for delivering a high standard of holistic nursing care whilst promoting and maintaining health education and independence. The role holder will ensure assessment and implementation or current and anticipatory care needs, ensuring the patients' needs are met by Neighbourhood services.
