Relief Rehabilitation Support Worker (Intermediate Care Team)
The Intermediate Community Therapy Team (ICT) is looking to recruit Rehabilitation Support Workers (RSW) into ICT on a relief basis. The team supports discharges from hospital, prevents admissions to hospital and provides rehabilitation in patients/service users’ own home, thereby regaining independence. The team works closely with colleagues in Health and Social Care and the voluntary sector.
Responsibilities
* Provide a highly varied range of support across therapy disciplines with guidance from the ICT team.
* Support discharge planning and community-based rehabilitation for patients/service users in their own homes.
* Collaborate with Health and Social Care colleagues and the voluntary sector to optimise patient outcomes.
Qualifications
* Minimum of 2 years experience working directly with patients/service users in the Health and Social Care setting
* Ability to work single-handedly and as part of a team
* SVQ III in Health and Social Care, HNC or equivalent
* Proven verbal and written communication skills
* Willingness to embrace the rehabilitation/re-enablement ethos and undertake further training and education as required
* Access to transport is essential; if driving themselves, hold a full driving licence. If unable to drive due to disability, adjustments to arrange transport will be considered
Location, Working Pattern and Contract
The ICT provides a seven-day service; the RSW must be willing to work shifts across the seven days and have use of a vehicle for community visits. This post will be based at The Balfour. The successful candidate will work on a Relief basis. It is a condition of employment that you must live and remain a resident within the UK for the post in which you will be employed with NHS Orkney.
Benefits And Salary
The salary for this post is Band 4 £30,353 - £33,016. If residing on Orkney, you’ll also receive a Distant Islands Allowance of £1,654 pro rata per year.
