Sensory Access Needs Assessor (Deaf & Hard of Hearing) Hammersmith & Fulham Council - Adult Social Care Rate: £21.52/hr PAYE | Hybrid: 2 office days home visits Contract: 4 months initial | DBS Enhanced | BSL Level 2 Essential Make a Real Difference Join our Sensory Team to empower Deaf, Hard of Hearing, and dually sensory-impaired adults in Hammersmith & Fulham. Conduct specialist assessments, craft person-centred support plans, and promote independence under the Care Act 2014. Key Responsibilities Carry out initial assessments & reviews for adults with sight/hearing loss. Design strength-based care plans, including direct payment options. Provide/demonstrate assistive equipment & collaborate with audiology teams. Advocate for service users & raise sensory impairment awareness borough-wide. Maintain Mosaic records & liaise with multi-agency partners. What We're Looking For BSL Level 2 (essential). Care Act 2014 & strengths-based practice knowledge. NVQ Level 3/A-Level equivalent; social care/sensory impairment experience. Excellent communication, IT skills (Mosaic training provided), & problem-solving.
