Community Mental Health Social Worker (Flow)
Based at Warwick Resource Centre
These are new posts generated by the modelling around the Enhanced Community Enablement Recovery and Rehabilitation Model; Proposal to Repurpose Hawkesbury Lodge.
The repurposed Hawkesbury Lodge will provide a recovery focused, patient centred, pro‑active and engaging model of residential rehabilitation care and support. This will include group and one‑to‑one activity focused upon supporting self‑efficacy recovery and maintenance of reduced clinical symptoms and greater/enhanced participation in activities of daily living to support transition to supported accommodation / independent living / access to own tenancy etc.
Main duties of the job
As a social worker in the team, you will receive clinical and professional support from a qualified Social Worker to maintain your professional registration. This support will be alongside the supervision and management that comes from the Trust, giving the best of both worlds.
The successful applicant will work alongside the leads and colleagues to continue to develop the service, pick up a relevant clinical caseload, provide evidence‑based interventions, and liaise with other agencies.
We are looking for enthusiastic, dynamic and experienced individuals who can work confidently on their own as well as being a team player. Good communication and problem‑solving skills are essential, as well as knowledge of mental illness and forensic awareness. We also seek people who can demonstrate the Trust's vision and values in practice, and who are motivated to be involved in innovation and service development.

Person Specification
QUALIFICATIONS
* Relevant professional qualification (e.g., RMN, DipCOT)
* Current unrestricted registration with HCPC/NMC
* Specialist knowledge across a range of procedures and theory
* Professional knowledge acquired through degree/diploma
* Relevant training and experience of specialist short courses
* Evidence of continuing professional development
EXPERIENCE
* Substantial experience in a community mental health care setting
* Developed specialist knowledge gained within a similar environment over a considerable period of time
* Significant experience of undertaking assessment, formulation, risk assessment and positive risk management
* Experience of safeguarding and investigations
* Proven experience of people management
OTHER
* Must have full driving licence and access to a motor vehicle for business use
