Were looking for a dedicated and forward-thinking Clinical Pharmacist to join our friendly, supportive team for one session a week. This is a fantastic opportunity to make a real difference in patient care while working in a collaborative GP surgery environment.

In this varied role, you'll take the lead on diabetes reviews, support our CQC compliance work, contribute to clinical audits, and help us continually improve the quality and safety of our services. You'll be a valued part of a multidisciplinary team that genuinely cares about innovation, patient outcomes, and professional growth.

Main duties of the job

Conduct structured diabetes and long-term condition medication reviews.

Support the practice team with CQC preparation, evidence gathering and compliance work.

Lead on and contribute to clinical audits to improve quality and patient safety.

Provide medicines optimisation advice to clinicians and patients.

Work collaboratively with the wider team to enhance patient outcomes and support service development.

Mayfield surgery are based in the heart of the Roehampton community and have a friendly and supportive practice team.

We are an enthusiastic and forward-thinking team of health care professionals and administrative colleagues, committed to multi-disciplinary team working and offering patient centered high quality care.

We use total a "Total triage" or "Modern general practice" pathway which ensure that all patient medical request are reviewed by a clinician, prior to appointments being offered. This means that we can ensure the patients are seen by the appropriate service.

The Practice uses Accurx and Docman for Workflow management. We also outsource medical report requests.

PMS Practice

Practice list size 6,800

3 Partners, 3 Salaried GPs, 1 Advance Nurse Practitioner, 1 Practice Nurse, 1 Nurse Associate, 1 Mental Health Liaison worker and 1 Drug and alcohol counsellor.

ARRS roles including Pharmacists. Social Prescriber, first contact physiotherapist, and Health and Wellbeing Coach

GP led long term condition clinics.

High QOF achievement

CQC rating of good across all areas.

Strong links with PRIME PCN, Wandsworth GP federation and Wandsworth ICB team.


Job responsibilities

Undertake structured medication reviews for patients with diabetes and other long-term conditions.

Provide medicines optimisation support, ensuring safe, evidence-based, and cost-effective prescribing.

Offer specialist advice to patients and the multidisciplinary team on the use and monitoring of medicines.

Support repeat prescribing processes and contribute to prescribing safety initiatives.

Quality, Governance & Audit

Lead and contribute to clinical audits to improve patient outcomes and practice performance.

Support the practice in CQC compliance activities, including evidence gathering, documentation, and quality assurance processes.

Contribute to service development projects and quality improvement initiatives.

Work closely with GPs, nurses, healthcare assistants, and administrative staff as part of a multidisciplinary team.

Communicate effectively with patients and carers to support shared decision-making and improve health literacy.

Participate in team meetings and contribute to the development of clinical policies and protocols.


Person Specification


Additional

* Experience of working in a primary care or clinical setting.
* Strong understanding of medicines optimisation and long-term condition management.
* Excellent communication and teamwork skills.
* Ability to work autonomously and manage own workload effectively.
* Experience of conducting diabetes reviews or other chronic disease management clinics.
* Involvement in clinical audits or quality improvement projects.
* Understanding of CQC standards and regulatory requirements in primary care.
