Responsibilities
* Build relationships with people who may struggle to access support for their health needs, proactively inviting people for an annual physical health check.
* Provide comprehensive physical health assessments (in both clinic and home settings) and signpost individuals to services to improve their physical health.
* Set goals with individuals to help improve their physical health and promote self-management.
* Work closely with existing teams in Primary and Secondary care to enhance provision and collaborate with Christchurch Community Mental Health Team, embedded within Primary Care GP Surgeries to address physical health care needs.
* Develop and promote physical health care goals and plans with individuals and relevant staff to support sustainability and advance self-management.
* Provide comprehensive physical health monitoring, assessment and annual review appointments for people diagnosed with a serious mental illness, aiming to reduce health inequality and mortality gaps.
Training, Development & Support
* High-quality training covering core mental health awareness, physical health promotion and behaviour change for people living with serious mental illness.
* Protected time to access relevant training and appropriate supervision & reflective practice; access to multidisciplinary team structures as part of continued development.
Work Environment & Collaboration
* Post holder will work closely with the Christchurch Community Mental Health Team but be embedded within Primary Care GP Surgeries to provide physical health care needs for people within primary and secondary care services.
* Work with individuals and relevant staff through the process to develop physical health care goals and plans to promote self-management and collaboration with key partner organisations and communities.
