We are seeking motivated, dynamic, and passionate Band 5 Community Nurses to work within our South Bedfordshire Primary Home Care Team, covering Leighton Buzzard, however, there is an expectation that staff will cover across the bases if needed. This is an excellent opportunity to develop your knowledge and skills within the community nursing setting. You will have the opportunity to be involved in facilitating self-care for patients with long-term conditions, health promotion and innovative activities and projects that enhance patient service delivery and staff wellbeing. You will also have the opportunity to be involved in Urgent Response and Virtual Wards, which is a focus of our teams

Detailed job description and main responsibilities
You must have a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a shift pattern covering 7 days a week from 08:00-21:00.
This is a full time, fixed term post for 12 months, until September 2026.

Essential criteria
1. Registered General Nurse
2. Demonstrate evidence-based practice and up-to-date clinical skills
Desirable criteria
3. Evidence of post-basic professional development
4. Post-registration qualification in a relevant clinical field
5. Relevant teaching/mentorship qualification or commitment to undertake
6. Post-registration nursing experience, including community experience
other
Essential criteria
7. Current UK driving license and access to a car
8. Ability to travel across sites and across Trust boundaries to meet patient needs and attend meetings, etc.

23. Occasional distressing or emotional circumstances such as imparting unwelcome news, care of terminally ill or safeguarding issues
24. Post holder could work in occasional unpleasant conditions during home visits and could come across unpleasant body odours, dust, noise/ Body fluid or even verbal aggressions
