Progressed Social Worker: NHS Continuing Healthcare

1 post (attached to Freeman Hospital Social Work Team but location may vary). 37 hours per week. Salary: N8 £40,777 - £45,091 per annum (progressed).
We currently have an exciting opportunity for a Social Worker who is interested in working with people who are being considered for eligibility for NHS Continuing Healthcare.
Responsibilities
* Represent Newcastle City Council in multi-disciplinary team meetings to consider eligibility for NHS Continuing Healthcare, both at initial assessment (DST) and at review. This work will not be limited to hospital discharge cases.
* Monitor and process Eclipse documentation to update records, and support plans where necessary, for people who are eligible for the fast-track pathway (this does not include arranging support or ongoing case management).
* Investigate and solve queries about FNC payments and NHS Continuing Healthcare payments, in partnership with colleagues in Business Finance.
* Act as a contact point for such matters with Newcastle ICB.
* Participate in retrospective reviews for NHS Continuing Healthcare.
* Identify when a recommendation is unfair and draft formal disputes under manager direction.
* Provide consultation and advice to social care colleagues about NHS Continuing Healthcare and joint packages of care.
* Prepare joint package of care proposals, and assist others to do so.
* Complete Care Act assessments and Mental Capacity assessments for people whose eligibility for NHS Continuing Healthcare ends.
* Devise cost‑effective and robust support plans in such situations.
* Maintain Excel data records for outcomes of DST meetings.
Qualifications
* You will be a progressed qualified Social Work Practitioner who has an interest in NHS Continuing Healthcare.
* You must have a relevant social work qualification such as a Degree in Social Work, DipSW, CQSW, or CSS, or equivalent, and must have current Social Work England registration.
* You will be a confident verbal and written communicator who is self-reliant, efficient and able to set and manage your own workload.
* You will be thorough and possess an eye for detail.
* You will have experience of multi-disciplinary working and be able to work collaboratively and confidently.
* You will have a strong commitment to professional responsibility in your practice.
