Job Summary
We’re looking for a compassionate and proactive Healthcare Assistant to join our Integrated Neighbourhood Team at Banbury Cross Health Centre. You’ll play a vital role in supporting complex, vulnerable, and often housebound patients across the Banbury area. This is a dynamic and rewarding position where you’ll manage a dedicated caseload, conduct home visits, and contribute to holistic, patient‑centred care.
About Our Team
Our Integrated Neighbourhood Team is a collaborative, multi‑disciplinary team (MDT) including GPs, Care Coordinators, Social Prescribers, community therapy services, and other healthcare professionals within the wider primary care team at Banbury Cross Health centre. We work in partnership with District Nurses, community therapy services, PML neighbourhood frailty and visiting teams, social workers, Hospital at Home, and Geriatricians to deliver integrated care. We also work with community and voluntary organisations such as Age UK, Dementia Oxfordshire, Cherwell District Council, and the Volunteer Transport Service to ensure our patients receive comprehensive support.
Main Duties of the Job
 * Deliver care to patients in their homes across Banbury (business‑insured vehicle and full UK driving licence required)
 * Support clinical colleagues with patient reviews and care planning
 * Perform clinical tasks such as phlebotomy, ECGs, blood pressure checks, and simple dressings
 * Recognise signs of deterioration and escalates concerns appropriately
 * Support patients and families with increasing frailty
 * Monitor long‑term conditions and promote health prevention and screening
 * Attend and contribute to MDT meetings
 * Maintain accurate patient records and assist with referrals
 * Uphold infection control standards and contribute to clinical governance
Qualifications & Experience
 * Experience delivering a healthcare service to others
 * Ability to provide quality care and demonstrate compassion and commitment to patients and carers
 * Good interpersonal, communication, organisational and time‑management skills
 * Good team‑working skills and ability to work independently following verbal or written instructions
 * Ability to recognise and refer when clinical advice is required and to follow protocols
 * Administrative competence in documentation, record keeping and use of clinical IT systems (EMIS, Blinx, Accurex, Docman, etc.)
 * Experience in using basic physiological recording equipment and accurate data entry
 * Understanding of confidentiality, data protection and clinical governance standards
 * Driving licence and personal vehicle (business‑insured required)
Administrative & Technical Skills
 * Accurate tracking and reporting of patient updates and care plans
 * Proficient in MS Excel, Outlook, Teams, and primary care IT systems such as EMIS, Blinx, Accurex, Docman (training will be provided)
 * Documentation of MDT minutes and support for administrative coordination
 * Familiarity with data protection and clinical governance standards

Salary and Contract
£25,000 to £27,000 a year (full‑time equivalent)
