Key Responsibilities: Deliver a professional social work service in line with statutory responsibilities and organizational policies. Promote independence and recovery through strengths-based and outcome-focused service delivery. Collaborate with other professionals and agencies to address complex needs and achieve optimal outcomes for individuals and their carers. Conduct high-quality assessments and develop support plans in consultation with service users and their carers. Ensure safe and timely discharges from hospital settings, supporting individual choice and independence. Utilize personal budgets creatively and flexibly to meet the needs of service users and their carers. Maintain accurate records using electronic systems and contribute to team initiatives and service development. Support and mentor newly qualified or less experienced staff and students. Participate in continuous professional development and maintain emotional resilience and professional wellbeing. Person Specification: Qualifications: SW, CSS, DipSW, or Social Work Degree; current Social Work England registration. Experience: Accepting referrals, assessing needs, managing caseloads, and working in social services or similar settings. Skills and Abilities: Knowledge of relevant legislation, effective communication, IT literacy, organizational skills, and the ability to work independently under pressure.
