Are you a Medicines Management Pharmacy Technician looking for a new role in a dynamic and patient focused environment? We are seeking an enthusiastic, dependable and self-motivated pharmacy technician with an interest in Healthcare for the Older Person to join our dynamic pharmacy team.
An ideal candidate will have initiative, a flexible and adaptable approach to work and an enthusiasm to develop their experience within the community setting. You will join the well-established Community Services Pharmacy Team based in Sidmouth Hospital to provide clinical and practical expertise to enable patients to take their medicines as prescribed and independently. You will provide medicines optimisation services to support inpatients as well as patients in their usual place of residence. This will include medicines reconciliation, clinical prioritisation for pharmacist review, support for discharge medications and providing medicines-related advice to professionals, staff and patients to help solve issues with adherence and the management of long-term conditions.
You will be working within multidisciplinary teams including prescribers, nurses, therapists and social care staff to support patients to manage medicines safely. You will work as part of a co-ordinated care team to deliver a high quality, person-centred medicines management and optimisation service.
Additionally, the wider team provides a clinical pharmacy service to Eastern Community Hospital intermediate-care inpatient units in Tiverton and Exmouth as well as a service to patients in their own homes (including in Exeter).
You must have excellent organisational and communication skills, with outstanding interpersonal skills. You must have an eye for detail, be a good problem solver and be able to prioritise your workload. The team will support you in this exciting opportunity for personal development. We will provide clinical in-house training and support to ensure effective use of our electronic prescribing and medical record system, EPIC.
You must be able to travel to meet the requirements of the role working in community hospital and visiting patient’s in usual place of residence.
Prior medicines management experience is desirable.

This role suitable for experienced technicians that already hold a Medicines Management Accreditation but also as a Band 4 to 5 progressive role for newly qualified technicians or technicians that are due to qualify in September 6 and are working towards their Medicines Management Accreditation.
