Community Staff Nurse

A vacancy has become available within the community nursing service in Halton. Qualified nurses with enthusiasm and motivation are invited to apply. The role involves providing high‑quality care to housebound patients, including palliative care, wound management, catheter maintenance, and phlebotomy. Candidates should possess the necessary skills to deliver patient care in home settings, demonstrate a commitment to working collaboratively within an integrated team, and be prepared to continuously develop their professional knowledge. Community nursing is advancing rapidly, and adaptability to change is essential. Flexibility, autonomous practice, and effective communication are key in ensuring safe, responsive community nursing services.


Main duties of the job

The post holder plays a vital role in supporting the District Nursing team through the management of an assigned caseload.

Key responsibilities encompass the assessment, planning, and evaluation of patient care needs. The incumbent works closely with the District Nurse to facilitate effective communication regarding patient care, maintain up‑to‑date care plans, and ensure seamless continuity of services, especially during reviews of complex care arrangements. Additional duties include contributing to mentoring, educational, and assessment activities. Collaboration with healthcare professionals and external agencies is integral to providing efficient and high‑quality service delivery.

Job responsibilities

Job Purpose

* Have responsibility for managing patients/clients within a defined clinical service/area, providing evidence‑based care and advice.
* Provide a nursing assessment of patients/clients; plan, develop and/or implement and evaluate programmes of care.
* Provide clinical supervision to other staff, students.
* May be responsible for routine work allocation and checking of tasks of support staff and/or less experienced staff.
* Communicate effectively with patients, staff and public using verbal, non‑verbal and interpersonal skills within everyday interactions, using strategies to address any barriers to understanding.
* Use manual and digital systems as effective communication tools with members of the health care team including for planned interventions and in times of escalation when supporting patient safety, contemporaneous record keeping and confidentiality.
* Plan, assess and evaluate programmes of care, including of a patient and or carer, according to changing health needs under the direction of the District Nurse, making informed decisions when deploying any responsible adjustments to an identified patient need.
* Deliver effective care and clinical skills including the safe administration and destruction of medicines, maximising patient dignity, comfort and safety.
* Utilise monitoring processes including of vital signs, taking timely intervention, action and/or escalation when supporting patient safety.
* Understand and use relevant patient pathways and discharge processes aimed at optimising the patient experience and outcomes, communicating and collaborating with other members of the Primary Health Care Team, voluntary and statutory organisations to support individual patient care.
* Communicate appropriately, during and following a patient’s transfer of care, with health professionals or support networks, maintaining a patient safety approach.

Practical Skills

* The ability to travel independently across the Trust.
* The ability to work flexibly in accordance with service needs.


Experience

* Range of extended clinical skills & demonstrate clinical knowledge.
* Venepuncture and immunisation.
* Able to communicate sensitive information to patients, relatives and carers.
* Able to assess, plan, implement & evaluate packages of care.
* Previous community experience.


Knowledge and Understanding

* Experience of using Electronic Patient Records.
* Audit and research.
* Supra pubic catheterisation.
* Tissue viability qualification.
* IV Therapy.
* Chronic disease management.
* Experience of working within palliative care.
* Teaching & assessing skills.

Qualifications

* Active NMC registered adult nurse.
* Nursing Qualification at level 5/6 study.
* Teaching and Assessing qualification.
