What you will do:

* Work closely with multidisciplinary teams and other agencies to deliver integrated care.
* Advocate for the rights and needs of individuals with learning disabilities and autism to promote inclusion and accessibility.
* Ensure all documentation and reports are accurate and up-to-date.
