What You’ll Do
* Lead and chair Multi-Disciplinary Team (MDT) meetings.
* Conduct CHC reviews and assessments virtually via MS Teams or Zoom.
* Communicate complex clinical information with empathy and professionalism.
* Produce clear, person-centred reports for both clinical and non-clinical audiences.
* Work with a Integrated Care Board (ICB) to improve CHC delivery.
What You’ll Need
* NMC Registered Nurse with at least 12 months post-qualification experience.
* Strong written and verbal communication skills.
* Knowledge of CHC Framework (2022), Care Act (2014), and Mental Capacity Act (2005).
* Proficiency in Microsoft Outlook, Excel, and remote working tools.
