Offer description

A Vacancy at East of England Community Health and Care NHS Trust (Cambridge). Are you a registered nurse or midwife? Have you an interest in working with children and young people aged 0-19? If you are passionate about children, young people, families and working as part of a team that makes a difference to health outcomes, then this opportunity is for you.

As a service, the child is central to what we do. We deliver early intervention, health promotion and build therapeutic relationships to empower families, children and young people to make the best choices.

The Public Health Nurse role is an opportunity to work within the Healthy Child Service and then, where opportunity arises, to apply for Health Visitor/School Nursing training.

You will be working within the skill mix delivering the Healthy Child Service in a variety of settings, including home and community venues. You will work closely with partner agencies and voluntary services to ensure that the right support is given at the right time to children, young people and families. You will identify needs and deliver support in conjunction with the wider skill mix, working with children on the Universal to specialist pathways, including safeguarding and supporting the most vulnerable families. You will deliver evidence‑based practice in line with the NMC code and maintain knowledge and skills as part of ongoing professional development.

* Work as part of a multidisciplinary 0‑19 team to identify the health needs of children, young people, families and the community, and contribute to developing a strategy to address individual and community needs.

* Develop core skills of assessing, planning, implementing and evaluating care to provide early intervention and ongoing care to families, children and young people.

* Support other localities as required, including Just One Number, to develop clinical skills and maintain effective working relationships and continuity of services.
* Promote and support families to access the Norfolk Family Hub offer.
* Use digital resources on Just One Norfolk during all contacts to enhance self‑care skills for children, young people and families.
* Take responsibility for own clinical practice, including supervision, and adhere to the NMC code.
* Contribute to the development of local policies and procedures, and to relevant service design projects.
* Utilise virtual platforms in assessments, interventions, training and networking to support effective practice and partnership working.
* Collaboratively facilitate the Pathway to Parenting (P2P) programme for expectant parents in the community.
