Clinical Lead - Advanced Specialist Nurse / Practitioner (Community) Hours: 37.5 Hours Per Week Salary : £44,533 - £50,961 - Depending on Experience and Qualifications Our Specialist Palliative Care Community Team has an exciting opportunity for an experienced Specialist Nurse/Practitioner to join our Community Team. We are seeking an enthusiastic and highly motivated experienced registered nurse or practitioner to join the multi-disciplinary team in the provision of specialist palliative care to patients and their families. The post holder will need to be able to work autonomously and be responsible for complex holistic assessments and planning care to meet patients’ practical, psychological, spiritual, and social needs. They will work closely with primary and secondary care professionals and Hospice colleagues, and good communication and team working skills are essential. Applicants should have palliative care experience and be in possession of or willing to work towards, a master’s. We would also consider a development post into the Advanced specialist role.
