Job overview
The Intermediate Care Service offers care and therapy support to patients in the Cambridgeshire and Peterborough health system following an acute hospital admission, to step down from community hospital or prevent admission.
The service runs seven days a week 0700 – 2200. The vision of the service is to provide outstanding, patient focused, community based health care and therapy, optimising peoples potential to achieve their goals by supporting earlier discharge from hospital as part of the HomeFirst model.
The job role is to provide leadership and management to the band 4 assistant care coordinators and band 2 and 3 integrated care workers working in the Intermediate Care Locality. Working in alignment with, and under direction of the Senior Care Co-ordinator, the post holder will ensure the smooth implementation of tasks and responsibilities assigned to care coordination regarding allocations, care planning, referrals, pending list management and patient flow.
Main duties of the job
* Lead and manage the care coordination team and enable collaborative working between care coordination, integrated care workers and the therapy and nursing staff.
* Provide leadership and management (in alignment with the supervision structure) to the team including appraisals, supervision, training and development of bands 2, 3 and 4 staff.
* Assist the Senior Care Coordinator and Intermediate Care Team Lead to monitor and report on quality, productivity, and patient flow in line with internal KPI’s.
* Visit patients in the community when required to ensure quality, support patient flow and manage issues related to the delivery of care.
* Deal with telephone complaints/enquiries and other communications with tact, diplomacy and confidentiality in line with CPFT procedures and policies.
* Lead and manage the recruitment for all integrated care workers and band 4 care coordination staff in line with processes and policies.
* Support the Senior Care Coordinator in the supervision, development and appraisal of assistant care coordination staff and integrated care workers.

Experience
* Experience of staff management or supervision of other staff.
* Experience of care coordination or equivalent case management or administrative role.
* Understanding of and/or evidence of working within intermediate care or discharge planning pathways.
* Evidence of successful working within multi-disciplinary teams.
* Evidence of managing workload and ability to prioritise.
* Evidence of supervision training and/or experience • Knowledge of safeguarding policies and procedures.
* Experience of working with wider stakeholder groups
Education/Qualifications
* Level 4 in Business Administration/ A Levels or equivalent acquired through experience.
* 5 GCSEs Grade A-C or 9-4 including English.
Skills & Abilities
* Knowledge of SystmOne
* Evidence of leadership ability, able to influence, involve and develop individuals and teams.
* Able to demonstrate commitment to high quality care and service provision
