Job Duties
The Virginia Department of Health (VDH) is dedicated to protecting and promoting the health of Virginians. The VDH is made up of a statewide Central Office in Richmond and 33 local health districts. The Chesterfield Health District provides public health services to the residents of Chesterfield County, Powhatan County, and the City of Colonial Heights.
The Chesterfield Health District is seeking an experienced and mission-driven Population Health Manager. The Population Health Manager provides or coordinates health improvement planning services in the local health district by planning, implementing, and evaluating health education activities and working closely with others to fulfill health improvement needs and goals. Typical duties may include: preparing and submitting grant applications related to health improvement projects, delivering educational programs to clients and staff, conducting health education trainings, revising and developing educational materials, collecting program-specific data and writing reports, conducting needs assessments and facilitating the CHIP process, providing technical assistance to local and community agencies/organizations regarding program planning, serving as public contact or PIO about services and activities and preparing news releases and coordinating district media contacts and activities. This individual will oversee and supervise the Population Health staff, support District strategic plan development in coordination with health district Director and will serve as a member of the district's Senior Management Team working closely with the Health Director and Managers to achieve the VDH mission.
Minimum Qualifications
Considerable knowledge and experience in the principles and practices of health education, community health assessment and improvement planning, and community organization and engagement
Experience performing research and utilizing quantitative and qualitative methods for assessing data and programs
Experience with project management, development of goals and objectives, work planning, and coordination of activities and initiatives
Experience with grant management, grant writing, and grant activity reporting
Experience preparing and delivering media updates, informational materials, and presentations to a variety of audiences
Excellent communication skills, verbally and in writing, to effectively manage interpersonal and interorganizational communication and coordination networks
Proficiency in the use of standard Microsoft Office products
Additional Considerations
Previous experience in public health and the assessment/improvement process preferred
Certified Health Education Specialist (CHES) for Health Educators preferred
Familiarity with practices and principles of organizational strategic planning
