Job Title: Support Coordinator
Type: Permanent, Full Time (37.5 hpw)
Pay: £13.21 per hour (£25,838 per annum) and great benefits including Health Cash Plan
Location: Sunderland

Typical Day as Our Support Coordinator:
* Creating person-centered support plans with customers, working with clinicians on Risk Management Plans.
* Supporting customers with independent living skills such as budgeting, cooking, and engaging with external services.
* Promoting Home Group’s values within your team.
* Collaborating with local authority, NHS mental health teams, safeguarding teams, and housing options teams.
What You Bring:
* Passion to support customers to live their best lives with attention to detail.
* Experience in coordinating and signposting to internal and external services.
* Ability to work on your own initiative, remain calm under pressure, and be resilient.
* A flexible, personal approach tailored to each customer’s needs.
Our Team: You’ll join Jodie, Senior Client Service Manager, and Michaela, Client Service Manager. We’re passionate about our work and proud of our unique service in Sunderland. We’re supportive and enjoy sharing cups of tea and chats.
Job Details:
* Flexible start and finish times to meet customer needs.
* Working a set rota pattern of day shifts, including weekends and Bank Holidays.
* Using technology to update spreadsheets, complete online learning, and collaborate with colleagues and stakeholders.
* Enhanced DBS check required, paid for by us.
