We are seeking a highly skilled and dedicated SEND Practitioner to join our Inclusion Team. This is a key role within the department, supporting the SENCO in the effective day-to-day management of SEND provision and ensuring high-quality support for pupils with additional needs. The successful candidate will be responsible for overseeing EHCP processes, coordinating provision, and working closely with pupils, parents, teachers, and external professionals.

About the Role
Working closely with the SENCO, you will help ensure that every learner receives the right support to thrive academically, socially, and emotionally. This is a hands-on, pupil-centred role involving the coordination of EHCP processes, management of SEND provision, communication with families, and collaboration with teachers and external professionals.

Key Responsibilities

1. Lead on the coordination and completion of EHCP paperwork, consultations, and Annual Reviews.
2. Maintain accurate provision maps and support the monitoring of SEND interventions and their impact.
3. Oversee the Assess–Plan–Do–Review (APDR) cycle for allocated pupils and ensure timely follow-up actions.
4. Play a leading role in transition processes, including Year 6–7 and mid-year admissions.
5. Act as a key point of contact for parents and carers, building strong and supportive relationships.
6. Provide advice and guidance to teaching staff on effective SEND strategies and classroom practice.
7. Support the planning, delivery, and evaluation of targeted interventions for pupils.
8. Work directly with pupils to support their academic progress and emotional well-being.
9. Collaborate with external professionals, primary schools, and internal teams to ensure coordinated support.
10. Assist the SENCO with day-to-day running of the SEND department and contribute to whole-school SEND initiatives.

The ideal candidate will be able to manage complex processes such as EHCPs and Annual Reviews, while also building positive relationships and offering practical support to colleagues. You will be proactive, adaptable, and committed to ensuring every pupil receives the right support to succeed.

We are looking for someone who has:

11. A solid understanding of SEND Code of Practice and statutory responsibilities.
12. Experience supporting pupils with a range of additional needs.
13. Excellent organisational skills and attention to detail, especially in managing paperwork and deadlines.
14. Strong communication and interpersonal skills, with the ability to build trust with families and staff.
15. A collaborative approach and willingness to work as part of a busy inclusion team.
16. A commitment to inclusive practice and improving outcomes for young people.
