An exciting opportunity has arisen for an experienced and highly motivated Registered General Nurse (RGN) or Registered Mental Health Nurse (RMN) to work as a CHC Nurse Assessor within the Northamptonshire area. This community-based post offers the opportunity to enhance assessment and decision-making skills in a challenging but supportive environment.
The posts are being recruited at an initial 6-12 month fixed-term contract.
As a Band 6 CHC Nurse Assessor, you will be responsible for determining eligibility for NHS Continuing Healthcare funding through assessments, reviews, and appeals, in accordance with the National Continuing Health Care Framework. You will ensure your patients experience a smooth and efficient CHC process, enabling timely provision of high-quality care. Building strong links with health and social care colleagues is essential for effective multidisciplinary working and understanding local NHS and independent sector services in individuals' homes and care homes.
Reporting to a Band 7 CHC Clinical Nurse Specialist, the Band 6 CHC Nurse Assessor will:

8. Have excellent time management skills and thrive in a fast-paced environment.
9. Be proficient in Microsoft Office with intermediate typing skills.
10. Be adaptable to changes at short notice.
11. Be willing and able to travel daily across the county and occasionally nationwide.

The role involves facilitating multidisciplinary assessments, supporting the team, managing cases, ensuring compliance with guidelines, and supporting personalisation and pathway delivery for NHS Continuing Healthcare. Additional responsibilities include investigations into eligibility challenges, acting as Duty Clinician when required, maintaining data systems, participating in professional development, and ensuring statutory training compliance.
The post holder will maintain professional relationships with internal and external stakeholders, communicate effectively, and provide guidance on CHC-related matters. They will also support training, undertake research, and perform other duties as delegated by their line manager, including assessments related to funding elements such as s117, joint funding, or ABI.
The role may include elements of dispute management, transitional bed processes, SEND, and safeguarding.
