What you’ll be doing?
In this role, you’ll play a crucial part in ensuring our teams have the tools, knowledge, and processes they need to deliver a high‑quality service. You will:
* Support vulnerable customers during onboarding, working with relevant teams to ensure a smooth transition into their home.
* Help residents sustain tenancies by identifying risks linked to vulnerability, disability, or social challenges and delivering targeted support to prevent tenancy breakdown.
* Carry out risk assessments in the home to ensure own and others personal safety.
* Coordinate proactive interventions in cases where behavioural concerns (e.g. ASB, hoarding, addiction, self-neglect) may threaten tenancy, ensuring early engagement and prevention of escalation.
* Conduct welfare checks where concerns have been raised by staff, neighbours, or third parties, assessing the resident’s situation and taking proportionate steps to protect their wellbeing.
* Provide direct support for complex cases, such as attending medical appointments with customers or helping manage hoarding issues—always within health and safety limits.
What you’ll bring?
We’re looking for someone with:
* Understanding of housing-related support needs, tenancy sustainment, and the impact of vulnerabilities such as mental health issues, domestic abuse, addiction, and self-neglect.
* Ability to assess risk and identify early warning signs that could impact a customer’s ability to maintain their tenancy.
* Confident in developing action plans with customers and tracking progress toward agreed outcomes.
* Ability to build trust and empathy with people facing complex challenges.
* Skilled at navigating sensitive situations, handling complaints, and responding to distressed or vulnerable individuals with professionalism and care.
* Confident in using housing management or case management systems (e.g. IPC, Dynamics, or similar) to record and manage casework and generate reports.
* Experience supporting vulnerable individuals or households in a housing, social care, health, or community support setting.
* Experience managing a caseload, including conducting home visits, coordinating multi‑agency support, and delivering positive outcomes for service users.
* Experience of contributing to or attending multi‑agency forums (e.g. MARAC, safeguarding meetings, case conferences).
* Full UK driving licence and access to a vehicle for work purposes.
* Ability to work flexibly, including occasional home visits outside normal working hours, in response to urgent welfare concerns.
