The Hospice Advice & Response Team (HART) provides specialist palliative care advice and rapid clinical response to patients, families and healthcare professionals, 24 hours a day, 7 days a week. The service plays a vital role in supporting people with life-limiting illness during times of crisis, ensuring timely intervention, compassionate support and coordinated care in the community. As a Clinical Nurse Specialist (HART), you’ll be responsible for delivering high-quality specialist palliative care through telephone triage and rapid face-to-face response. This role requires advanced clinical skills, sound judgement and the ability to respond calmly and effectively to complex and often unplanned situations. You’ll work as part of an experienced multidisciplinary team, supporting patients to achieve their preferred place of care and helping to reduce unnecessary hospital admissions. This role is ideal for a registered nurse with experience in palliative or end of life care who is confident working autonomously, values person-centred care, and is passionate about making a meaningful difference at some of the most important moments in people’s lives. Key Responsibilities § Provide specialist palliative care advice and support to patients, families and healthcare professionals via telephone and face-to-face visits§ Undertake timely triage assessments and determine appropriate response times§ Deliver holistic assessments addressing physical, psychological, social, cultural and spiritual needs§ Manage complex symptom control and palliative emergencies, escalating appropriately§ Coordinate and contribute to high-quality care planning and interventions§ Work collaboratively with internal teams and external healthcare partners§ Maintain accurate, timely and confidential clinical documentation§ Contribute to audit, data collection, service evaluation and quality improvement§ Support education and training of colleagues and external professionals§ Act as an advocate for patients and families across the care pathway About You We’re looking for a compassionate and skilled nurse who thrives in a fast-paced, responsive service and is confident managing complex clinical situations. You’ll bring: Registration as a Registered Nurse At least 3 years post-registration experience Proven experience and knowledge of palliative and end of life care Experience in urgent clinical decision-making Strong communication and interpersonal skills Ability to work autonomously and as part of a multidisciplinary team Sound knowledge of symptom management and clinical guidelines IT skills and confidence maintaining electronic patient records A full driving licence and access to a suitable vehicle Advanced communication skills training, independent prescribing or healthcare assessment qualifications are desirable, but development will be supported.
